After the Oracle database has had an unexpected failure which was subsequently fixed, the RSA Identity Governance & Lifecycle application and AFX remain running and were never shut down. This RSA Knowledge Base Article describes what should be done in this situation to properly recover from an unexpected database failure.
 
The safest approach and best practice to recovering from this situation is to restart AFX, RSA Identity Governance & Lifecycle and the Oracle database. This is true regardless of whether or not the RSA Identity Governance & Lifecycle user interface is accessible. It may be accessible but unexpected and undesired behavior could result.

Login as user oracle and execute the following commands:
 
afx stop
acm stop
acm stopdb
acm startdb
acm start
afx start
