IssueNotifications fail to install during a package installation with the below error in the package installation log :

Upon inspecting the subject lines of the failed notifications you will notice that they will have an extra '[' and ']' around the field references, as shown in the example below.

Summary of meeting - [[Meeting Date]]

Here 'Meeting Date' is the field reference and you can notice the additional brackets around it.
ResolutionTo resolve the issue, remove the additional square brackets around the field references in the subject line of the notification and re-generate the package.
