IssueCustomer gets "Failed to generate QR code" error when trying to activate his software token using QR code in Self-Service Console.

CauseActivation Code Expiration of the token is set to 'Do not expire activation codes'.
 
WorkaroundSet an expiration date for the token activation code by following the below steps:
1. 
Log into the Security console and navigate to Setup >>System Settings.
2. Under Authentication settings, Click on Tokens.

3. Under 
Dynamic Seed Provisioning Configuration, Enable Activation Code Expiration and Set the number of days that the activation code can be used after the software token is distributed (default 7 days).
4. Click Save.
5. Redistribute the token.
