IssueWindows Application Event Log indicates the application pool generated a General Protection Fault:

W3SVC Warning None 1009 N/A server A process serving application pool 'DefaultAppPool' terminated unexpectedly. The process id was '8000'. The process exit code was '0xc0000005'.
The RSA ClearTrust Agent log file does not log any additional events after the time when the application pool is terminated. The last event in the ctagent.log file is:

<Debug> Heap Reset
After authenticating at the RSA ClearTrust authentication screen only part of the protected ASP page is displayed
Crash dump shows general protection fault accessing SERVER_NAME variable
CauseIn certain instances where an ASP script is pulling content from an XML file, the RSA ClearTrust Agent may prematurely release memory allocated to an established ISAPI request
ResolutionThis issue is resolved in hot fix 4.6.0.100 for RSA ClearTrust Agent 4.6 for IIS 6.0. Contact RSA Customer Support to request this hot fix, or request the latest fix level (which is cumulative, and contains fixes from previous fix levels). Review the provided Readme file for installation instructions.
