IssueThis issue is documented in RSA Governance & Lifecycle 7.0.1 as a known issue:
Testing has found that when the system is configured to "Disable accounts in request" is enabled in a Termination workflow, only one change item is generated when a user is terminated.
The termination should generate two items, one for disabling the account and the other for terminating the user.  While testing the Access Fulfillment Express (AFX) connectors for Lotus Notes and Google Apps we have found that for these two connectors only a single change is generated and the disable event is not present.

CauseThis is currently listed as a known issue in the 7.0.1 release notes. The cause for this behavior is not currently available.
ResolutionThis issue is known by RSA Engineering and a fix is not currently available at the time of this KB being authored.
