IssueActivity is not created for a change item in a Change Request.
For example: If a Change request that was submitted contains 3 items, but only 2 activities are created.

CauseWhen a AFX workflow is configure to automate the provisioning on the end point AND fulfillment is completed successfully by AFX, therefore no manual activity is needed/created. 
ResolutionThis expected behavior/Functioning as designed.
Usually, ACTIVITIES are created when Manual Fulfillment is used. The Fulfillment Phase is setup to run "Manual Activities" or "Manual Account Activities" Workflow and these create activities for the change items being processed.
Another common activity is the Activity created from "Default AFX Manual Fulfillment Subprocess" of which is used as a "failover" option of the workflow, in case AFX fails. 
Note that an Fulfillment activity can be closed automatically by the system in certain conditions. The most common condition is when the entitlement is a local ROLE or Local ENTITLEMENT.
