IssueThis issue occurs usually after a fresh installation and the login screen will seem to be off centered toward the upper left corner of the screen.
Attempting to log in as the sysadmin and verification of the password being reset has been established, the user gets a login error stating "The username and password are required".
CauseThis is an IIS web tier issue caused by ISAPI filters being applied incorrectly or there are additional filters that need to be removed.
Resolution1. Verify pre-requisites are installed correctly via the Server Manager Add Roles and Features
Under Server Roles > Web Server (IIS) > Web Server > Application Development
-Verify that ASP.NET4.5 is installed and that ISAPI Extensions/ISAPI Filters are both installed

2. In IIS either on the site level or the Default Web Site level (depending on how the website structure was installed),
under the IIS section in the center pane, locate the ISAPI Filters and double click to open.
-Verify that there aren't any additional filters besides ASP.Net_4.0_32bit and ASP.Net_4.0_64bit

After removing the additional filters, perform a re-registration of ASP.NET4.5 and then open up the Command Prompt as an administrator then perform an IISRESET
