Account Template for Role Causes Invalid Account Name in CR for RSA Identity Governance & Lifecycle

Issue
Account names in change requests are not displayed properly when generated from a Role member add, when a Role with entitlements for an application that has an account template with an account create form attached. Removing the form from the account template resolves the issue. The Account name is ending up in the form: "ACCOUNT_NAME account on APPLICATION_NAME".

Cause
The Account Template should not be associated with any Role or Rule changes, if it has mapping to any Request Form.

Resolution
This issue has been resolved in versions 7.0.2 P10, 7.1.0 P03 and 7.1.1 (see ACM-83004).  Please upgrade to one of these versions or higher.  The fix in the upgrade also automatically removes invalid mappings.
Workaround
Remove the Account Creation Form from the Account Template that is being associated with a Role or Rule.
