"Invalid column index" error in RSA Identity Governance and Lifecycle review "Total Entitlements not in Review"

Issue
When viewing the Review Results for a review and clicking the Application Coverage button and then selecting the Total Ents Not in Review option, the following exception is generated:
 
Caught SQLException: Invalid column index

In addition, the /home/oracle/wildfly-8.2.0.Final/standalone/log/aveksaServer.log lists the following log message: 
11/04/2016 02:12:19.394 ERROR (http-0.0.0.0-8080-2) [com.aveksa.server.report.DataSource] Could not create DataSource
java.sql.SQLException: Invalid column index
Cause
This issue occurs in the following versions of the product.
  • RSA Identity Management and Governance  6.9.1,
  • RSA Via Lifecycle and Governance 7.0.0, and
  • RSA Identity Governance and Lifecycle 7.0.1.
Resolution
This issue is resolved in the following patches:
  • RSA Identity Management and Governance 6.9.1 P21,
  • RSA Via Lifecycle and Governance 7.0.0 (please upgrade to 7.0.1 version and apply the 7.0.1 patch),
  • RSA Identity Governance and Lifecycle 7.0.1 P02.
