Issue
The create account item is not getting created in the request for the applications/directories having the account template associated and Entitlements Require account enabled.

The following steps will reproduce the issue:
  1. Create an explicit access request.
  2. Select an entitlement from an application that requires an account.
  3. Move to the submission page.
  4. Click back and forth between the submission page and the previous page.
  5. Finally, submit the request.
  6. You will see the request does not contain the Create account item, though the local account is created in application/directory.
  7. Due to this, the CR will remain  stuck in the Wait for dependency state.

Cause
Clicking on the back button was not deleting the pending account created in the system and that is the reason it does not prompt to enter the details to create an account on endpoint. This will cause system to generate a CR only to add entitlements to an account, which will never get fulfilled as there is no account for the user on the endpoint.
Resolution
This is a bug fixed in 6.9.1 P18, 7.0.0 P05, 7.0.1 P01. Upgrade to any one of this versions to have the issue resolved.
