The RSA Identity Governance & Lifecycle request button for a form with IS_TERMINATED=1 returns no rows

Issue
A request button for a form with IS_TERMINATED=1 in the Changes apply to field is unable to display terminated users.  For example, here is a request form with IS_TERMINATED=1 in the Changes apply to field:

When the request button is created, the field Include Terminated Users is visible:

Once the action Go to a form is selected, the field Include Terminated Users is no longer displayed:
Create New Button after "Go to a form"

When the request button is used, no users with a value IS_TERMINATED=1 will be displayed, even though it is explicitly specified in the form.
Cause
This is a product defect and has been reported in engineering ticket ACM-76461.
Resolution
Defect ACM-76461 has been fixed in version 7.0.2 P07.
If necessary, use workaround for this issue as outlined below.
Workaround
When creating the request button, please do the following:
  1. Tick Include Terminated Users first.
  2. Select the action Go to a form and select the form with IS_TERMINATED=1.
  3. Note that the Include Terminated Users field will disappear, but it is still in effect.
  4. Create the button as normal, by entering in the rest of the fields.
