The RSA Identity Governance & Lifecycle request button for a form with IS_TERMINATED=1 returns no rows

Issue
When the request button is created, the field Include Terminated Users is visible:
When the request button is used, no users with a value IS_TERMINATED=1 will be displayed, even though it is explicitly specified in the form.
Cause
Resolution
If necessary, use workaround for this issue as outlined below.
Workaround
- Tick Include Terminated Users first.
- Select the action Go to a form and select the form with IS_TERMINATED=1.
- Note that the Include Terminated Users field will disappear, but it is still in effect.
- Create the button as normal, by entering in the rest of the fields.
