Account history is Missing
There are some orphan and active accounts identified in the system with no account history. Account history shows no transaction entry at all. Can someone please advise on possible root cause of this scenario?

Can you elaborate more on what do you mean by account history? Maybe some screenshots from the UI for those missing history records?

Account history available in UI for each account .
Application-> Account -> Account details
Attached is the screenshot for sample.
[Just to add, the account was mapped and re-orphaned at some point but these details are also no available in account history. Also the account Orphan date shown the first date the account was orphaned and not the recent orphan date]

I see that the created on column states that the account was created 3 minutes before being orphaned (might actually be the difference in time of processing between collecting/inserting the account itself as an object & the collecting/inserting the orphan status/dates since no mapping existed in the collection data).
Is the created on date here really accurate? if not, was there any database deletion statements done on account & account mapping tables in the IG&L database in the past?

I suspect that this account may have been deleted at some point. A deleted account is only soft deleted in our database but the fact that an account becomes a deleted account does change the way that account is handled. I am not sure how this affects account history but I suspect it does.

I've already tried deleting the account from the end source and bring it back again via collections and the history records were preserved and not whipped out and later changes were appended on the older ones in that account history table.

What version and patch are you reporting this issue on.
There were major changes to this area of functionality in recent patches.
I will have to research further to provide you with an explanation of this behavior.

I agree. We have seen this scenario with other accounts, where account gets deleted but history is preserved.

I don't believe any deletion was done on account /account mapping tables.
I have other example (from the same application) where account creation date and orphaned date are few minutes apart and the account history is fully available . Also some of these accounts were reopened multiple times and all transactions are visible in history as well as the orphan on date is correct and latest.
However there are some accounts (in same application) where no history is available and orphan on date is incorrect and the very first date (when the account became orphan for the first time) .
