How to pull reports for request forms

We have created a request form called "XYZ mandatory leave". Now I want to pull this request form report from Reports -> Tabular -> Create Report, but unable to do so as there is no option to pull reports for request forms. I think we need to write a custom query for this. If its a custom Query Any help on this issue is greatly appreciated.
