JavaScript To Redirect from Form

I'm creating a redirect in one of our Access Request forms. it is working but I need it to trigger on a specific radio button.  We need help with the IF statement to this script:

The radio button is named 'ReqReason' and the value is 'Change'.

 

Using V7.0.1 P2

 

5/2: We ended up using this command to add a link to the form that the end user needs to click to be redirected.
