Second escalation workflow/email not triggering

I have a customer that has just set up a number of escalation workflows/emails to be triggered if a reviewer does not complete a user access review in a given time period.  After six days the first reminder email is sent.  This one works without any problems.  However, if after two weeks the reviewer has still not completed the review a second reminder email should be sent. Checked email log, there are no entries for the second reminder email so it isn't being triggered.  Checked the review definition an results, checked escalation workflow.  Can't find any reason why email should not be triggered.  Any help would be appreciated.
