Issue
When a request is created using the form, the email body is not displaying the ${OptionalComments} value.  For example, 
  1. Add the comment variable in the email node for the supervisor or fulfillment workflow.

  1. Create a form involving the same workflow with random fields.

  1. Generate a Change Request using the form.

  1. Add a comment while performing the request.

  1. Check the email log and you will not be able to see the texts in the comment field.

Cause
The query to get the comments did not include the Request Form Change Request Item type.
Resolution
Upgrading to V7.0.01 P02. will resolve this issue.
