Issue
Manager Name is not adding which is in CSV form.

 
Cause
This is by design.
Workaround
As discussed, manager account need to exist in AD in order to set the user account as manager. This is by design in AD and we confirmed this behavior by testing the user that has account vs that does not have an account in AD (users collected from CSV file).  You can add a solution to the custom workflow in order to check if the user selected as a manager in the form has an account in AD and provision that first before setting that account as a manager and then provision the new user account.  

Another option would be to first provision the accounts in AD, for ALL users that are collected from the CSV file using the AFX Connector.
