RSA Identity Governance and Lifecycle 6.9.1 and higher request stays in an Open state when trying to delete roles

Issue
When deleting roles from the UI, the resulting change request stays in an Open state and no processing workflow is generated, as shown here:

Cause
We were keeping the indirect change item operation as Delete, when the role is deleted. As a result, when the Entitlement Require Account is set to Yes in Resources > Application/Directories > Request, we see exceptions and no processing workflow is formed. As a result, the CR remains in the Open state.

Resolution
Upgrading to V7.0 P05, V7.0.1 P02 or V7.0.2 will fix the issue.
Workaround
As a workaround, change the parameter ofEntitlements Require Account to No under Resources > Application/Directories > Requests.

This will allow the deletion of roles without exceptions and errors.
