Requests do not show any Fulfillment Phase details under the Status for the request in RSA Identity Governance & Lifecycle

Issue
On the RSA Identify Governance & Lifecycle Requests details page the requests do not show any Fulfillment Phase details under the Status for the request.  If you expand the "+" (plus) icon next to the Fulfillment Phase details a "-" (minus) icon shows but no Default AFX Fulfillment URI is available and you cannot see the status of the fulfillment details or click through to the fulfillment workflow.  The request otherwise appear to be processed normally.

Cause
This issue may occur if the workflow is damaged either through a metadata import and export or some other means and is missing the workflow Status variables required to retrieve the workflow status.

Resolution
Edit the RSA Identify Governance & Lifecycle Fulfillment workflow associated with the request and ensure it has correct values under the Workflow STATUS area for the following variables.
 
STATUS variableValue
Planned(Definite) ${job.name}
Executing${job.name}
Completed${job.name}

The STATUS values should look like this:

Workaround
Alternately, if there is no requirement for a custom workflow, use the default workflows shipped with the product.
