Resolution
AM 8.x Fix:
Login to Security Console and Navigate to Setup>Self-Service Settings> Click "Manage Authenticators" link under Provisioning.
Scroll down to the iPhone 2.0 profile and change the Image location to "iPhone_1.3.gif"
Click the Save button at the bottom of the page.
AM 7.1 Does not currently have a Workaround.
