Hi All, we are on Version 8 Patch 8 with a local database and are experiencing intermittent issues with both Direct and Indirect (RBAC) requests being marked as "Completed" but changes within the request remain "Pending Action" when they should have been fulfilled by AFX. There is no error that I can find and no manual-activity was created. It seems to happen when a Create Account is required first, resulting in the change to be marked as "Waiting On Dependencies", then moving to "Pending Action" when the Account is collected. There doesn't appear to be any call to AFX and I can find no errors involving AFX. I want to say it's happening in the Workflow, as if the Watch isn't being created or perhaps acted upon.

The workflows involved are mostly reliable and have not been modified in years and are pretty simple. Here is the Fulfillment Workflow screenshot:

I tried tracking down the Workflow processing path but so far, it's been challenging. Does anyone have any experience or advice?
