Challenge questions changes to different questions that are not chosen after clicking on forgot password in RSA Governance & Lifecycle

Issue
When a user tries to login and clicks on 'forgot password', the user gets different questions which are not part of the selected challenge questions.
As an example
1- A user selected the below two questions and added the answers using the below steps:
Login --> Users tab --> My passwords --> Challenge Questions.
2- User logged out and clicks on forgot password, then the user enters the account name and choose the right authentication source.
User got the different below question that is not chosen.
Resolution
To resolve the issue, please follow the below steps:
Log in with AveksaAdmin, navigate to Requests Tab --> Password Management --> Identity Confirmation --> Edit.
in 'Primary User Attribute', choose User Name Or Account Name instead of choosing one of them only.
