Review escalation workflows sometimes do not trigger in RSA Identity Governance & Lifecycle

Issue
When a review is generated and changed to review state Active via a review escalation workflow, other review escalations configured on the review do not trigger. In addition, although the review state is set to Active, the change in review state is not reflected in the review history.

If the same review is generated and has its review state manually set to Active, the other review escalations trigger as expected and the change in review state is reflected in the review history. 

For example,
  1. Create a rule definition that is initially set to On Hold, changed to Active by the Active Review workflow, and has multiple review escalations.

Cause
This is a known defect reported in ACM-87438.
Resolution
Fixed in 7.1.0 P05 and 7.1.1 P01.
Workaround
Manually change the review state from On Hold to Active.
