Issue
RSA Identity Governance & Lifecycle fails to initialize showing the below error on the GUI

Initialization has failed!

Unable to initialize security model. com.aveksa.server.core.DuplicateNameException: Aveksa ADC
The aveksa security application must be initialized first by the System Operations Node(SON).
See documentation regarding server nodes and deployment.

Cause

The failure occurs when a new Account Collector or Entitlement Collector is created and mapped to the Aveksa Application.   

The Aveksa Application Account and Entitlement Collectors are created automatically by the system and cannot be created, edited or deleted.

Resolution
This will be resolved in a future version or patch that will prevent creating new collectors for the Aveksa application.
 
Workaround
Please contact RSA Support and mention the KB article number for assistance with resolving the issue.  The Support team can provide a workaround to resolve the issue.
