Resolution
In this case, reinstalling AFX by downloading the AFX server archive resolved the issue. Downloading the AFX server archive and reinstalling AFX frequently resolves AFX issues and is always a good first step to try when troubleshooting AFX issues. Downloading the AFX server archive downloads all the AFX data from the database and restores the flat files to disk.
